What’s my first visit like?

We want your first visit to feel easy. A few things to know in advance:

  1. Arrive about 15 minutes early. This gives us time to check you in and review your paperwork.
  2. Bring: your driver’s license, insurance card, a list of current medications, and any prior medical records you can.
  3. Complete your registration in advance. Download and fill out the form, then bring it with you — or arrive 30 minutes early to complete it in the office. Download the Patient Registration form (PDF).
  4. If you’re scheduled for a physical, fast for 12 hours beforehand (water and black coffee are fine). If you are diabetic or have any condition where fasting may be unsafe, please call us before your appointment.
